    Dolby Vivisol aims to optimise the quality of life for patients requiring home oxygen, by providing a quality service underpinned by knowledge, safety and experience. We aim to work closely with clinicians involved in patient care to achieve optimal prescribing and clinical effectiveness.

    Ordering Oxygen

    There are two types of Home Oxygen Order Form (HOOF). The HOOF Part A should be used where the request is made by a non-specialist Health Care Professionals, or for temporary supply pending a specialist review. The HOOF Part A can only be used to order a concentrator or static cylinders for use within the home.

    The HOOF Part B should only be completed following a formal assessment by specialist clinicians. The HOOF Part B allows for ambulatory oxygen equipment to be ordered alongside any Long Term Oxygen Therapy (LTOT) Equipment.

    For assistance in selecting appropriate equipment for your patients on the HOOF Part B, please follow the link to access the Equipment Selection Tool (EST).  Please be aware, this is guidance only and you should use your clinical assessment when making your final decision.
